Private posterior polar cataract surgery in the UK typically costs around £3,200 to £5,000+ per eye in 2026, higher than a standard cataract (from about £2,900) because the opacity is stuck to a thin, sometimes deficient posterior capsule, raising the risk of capsule rupture and demanding a modified, unhurried surgical technique from an experienced surgeon. Most quotes are given individually after assessment.
What is a posterior polar cataract?
A posterior polar cataract is a well-defined, often dense opacity at the very back (posterior pole) of the eye’s natural lens. It is usually congenital or develops early in life and is frequently found in both eyes. Its importance is not the cloudiness itself but its close attachment to the posterior capsule — the delicate membrane that holds the lens implant in place. In a proportion of cases the capsule behind the opacity is thin or already has a small defect, which is why this cataract carries a materially higher surgical risk than a routine age-related cataract.
How much does posterior polar cataract surgery cost?
Because it is a complex cataract, pricing is usually individualised. As a 2026 UK guide:
The higher fee reflects longer theatre time, meticulous planning and the experience required — not a different implant. Compare standard pricing on our cataract surgery cost page, and premium lens options such as the EDOF lens or trifocal IOL. Femtosecond laser (FLACS) is sometimes used to create a precise capsulotomy in complex eyes.
Why an experienced surgeon matters
In posterior polar cataracts the standard step of ‘hydrodissection’ (injecting fluid to separate the lens) is deliberately avoided, because it can burst the weakened capsule. Instead the surgeon uses gentle hydrodelineation, low-flow phacoemulsification and a careful ‘inside-out’ approach, keeping the back of the lens untouched until last. Should the capsule open, an anterior vitrectomy and a lens placed in the ciliary sulcus may be needed — so having a surgeon who performs complex cataract and, ideally, has femtosecond laser and vitreoretinal support available is the single most important factor in a good outcome. Request a specialist assessmentWhat’s included in the price?
A transparent private package should cover the consultant assessment and biometry, your intraocular lens, the day-case surgery and theatre fee, and all routine follow-up and eye-drop medication. For a higher-risk cataract, confirm that the quote allows for any additional steps that complex eyes may require. If clouding of the capsule develops later, this is treated with a quick YAG laser capsulotomy.
How to be seen privately
You do not need a GP referral to pay privately. You can self-refer, choose a surgeon experienced in complex cataract, and usually be assessed within days. Eye Surgery Clinic provides consultant-led care and clear written quotes, with a network of experienced surgeons — see our surgeons. NHS surgery is also available; national waits average around 18 weeks — see NHS cataract waiting times by region.
