Refractive lens exchange (RLE) replaces the eye's clear natural lens with a permanent intraocular lens (IOL). Because the natural lens is removed, a cataract can never develop in that eye — so RLE both reduces your dependence on glasses and removes any future need for cataract surgery. At our partner clinics, RLE starts from £3,000 per eye for a monofocal lens, around £3,500–£4,200 for EDOF, and £4,000–£5,000 for trifocal or multifocal IOLs, all-inclusive.
How lens replacement prevents future cataracts
A cataract is the gradual clouding of the eye's natural crystalline lens — a normal part of ageing that affects around half of people in the UK by age 75. The one certainty about cataracts is that everyone who keeps their natural lens long enough will eventually develop them.
Refractive lens exchange removes that natural lens entirely and replaces it with a clear artificial intraocular lens. An IOL is made from inert acrylic or silicone and does not cloud the way a living lens does. Once your natural lens has gone, there is nothing left to form a cataract — the eye is protected from cataract for life, and you will never need cataract surgery on that eye.
This is why RLE is often chosen by people over 50 who are becoming long-sighted or presbyopic: it corrects the vision they have now and closes the door on cataracts later. What happens during the procedure
Lens replacement is performed under local anaesthetic eye drops. You stay awake but feel no pain — only mild pressure and light. Each eye takes 15 to 25 minutes, and you are at the clinic for around 2 to 3 hours in total.
- Numbing drops are placed in the eye and the surrounding skin is cleaned with sterile solution.
- The surgeon makes a self-sealing 2.2–2.8mm incision at the edge of the cornea — no stitches needed.
- Phacoemulsification gently breaks up and removes the natural lens through that incision.
- Your chosen intraocular lens is folded, inserted and unfolds into the capsular bag, where it locks into place permanently.
- The eye is shielded and you rest for 30–60 minutes before going home.
Recovery week-by-week
Most patients notice clearer vision within a day, but full healing takes about a month.
Day of surgery
Vision is hazy for a few hours. Eye shield worn the first night. No driving or heavy lifting. Eye drops begin.
Days 1–3
Vision clears noticeably. Mild grittiness or watering is normal. Most return to gentle activities and reading.
Week 1
First post-op review. Most patients are back to driving, working and walking — no swimming or eye rubbing yet.
Weeks 2–4
Vision continues to refine. Drops continue. Any new glasses prescription is finalised at week 4–6.
Beyond a month
Final vision settles. The treated eye is now cataract-proof for life. Second eye scheduled if both are being treated.
